Despite many advances in surgery, the quest for uneventful healing of the intestinal anastomosis after colon and rectal resections remains a challenge. The hospital stay was a month in ICU and a week+ in recovery. In Jan of 2011, the operation to remove an obstruction due to adhesions and hernias resulted in four operations due to the leaks and infections. But they are often caused not by negligence or technical error but instead by bacteria in the bowel that elude antibiotics, according to new evidence about this devastating complication of gastrointestinal surgery.
The scientists searched for the cause of these leaks by performing colon resections on healthy rats, removing 1 centimeter of the lower colon, then reconnecting the two adjoining segments.
This approach diverts stool away from the newly formed intestinal connection, giving it a chance to heal, but it can be distressing to patients and requires a second operation to restore intestinal continuity.Such leaks, which can develop days or weeks after the procedure, allow the bowel’s contents to spill into the abdomen and can cause pain, fever, sepsis and even death. The electronic medical record was queried for postoperative vital signs and laboratory studies, which were digitally abstracted. Introduction: The reported acceptable incidence of anastomotic leak following a colon resection is reported to be approximately 3-6%.
Even sustained aberrant vital signs and/or leukocytosis are not necessarily suggestive of a leak or other postoperative complication. We use cookies to help provide and enhance our service and tailor content and ads. After a surgeon removes a portion of the colon, she sutures the two loose ends of the colon together with stitches. In order to identify the bacteria that cluster at the surgical site and determine their role in causing leaks, the researchers plan to monitor patients via colonoscopy for up to three weeks after surgery, to observe daily how their anastomotic tissues heal. Of all the bacteria at these sites, E. faecalis stood out as the dominant microbe. Even in “uncomplicated” recoveries, tachycardia and tachypnea were almost routine, occurring in more than half of the patients frequently throughout the postoperative period. We aimed to profile the postoperative courses of patients who undergo intestinal anastomosis and determine how reliably abnormal vital signs predict anastomotic leaks.Consecutive patients undergoing bowel resection with anastomosis at an academic medical center from July 2009 through July 2011 were identified from a prospective complication database. This E2 strain began degrading collagen much sooner and was more virulent. When the researchers exposed rats to each of the two strains via enemas soon after surgery, those that received E2 all developed leaks at the surgical site within six days. Abnormal vital signs are often cited in retrospective peer review and medicolegal settings as evidence of negligence in the failure to make an early diagnosis. Then they used genetics to track the bacteria found at the surgical site as they evolved over time. The leaks appeared to be caused by the breakdown of collagen in the healing intestinal wall, so the researchers looked for bacteria that might make enzymes capable of collagen degradation.
The amount of bowel removed depends on the reason why this surgery is needed.
Abnormal values were defined as temperature >38°C, white blood cell count ≤4,000 or ≥12,000 cells/uL, systolic blood pressure ≤80 mmHg or diastolic blood pressure ≤50 mmHg, pulse ≥100 beats per minute, and respiratory rate ≥20 breaths per minute.
Despite numerous studies reporting the rate of anastomotic leak, only limited knowledge of its risk factors is available today. Abnormal vital signs are often cited in retrospective peer review and medicolegal settings as evidence of negligence in the failure to make an early diagnosis. I ended up with an Ileostomy. None of the rats receiving strains that produced negligible amounts of collagenase produced a leak. The choice of antibiotics and how they were delivered also affect the risk of a leak.
